Medibank Private Allocated Provider Number

Provider numbers for this category are allocated by Medicare Australia. For recognition of new practices or alterations to addresses of current practices, Medicare Australia can be contacted on 132 150 or via www.medicareaustralia.gov.au/providers/forms/medicare/ancillary_providers.htm.

Medicare Australia will then notify all health funds of the provider number and the relevant practice information.
